Before filling out this form please see chapter 23 of the Code of the Village of Shoreham regarding short term rentals.
This form is not electronic yet. It has to be downloaded, printed, and filled out by hand. Once that's done you can upload it (by scanning or taking a picture) and mail it to the Village Clerk.
